Destination Background
East and Southern Africa remain among the last regions of the world where visitors can experience spectacular wide expanses and free-roaming animals in their natural state. The African Safari has long been the dream vacation for many Westerners. Individuals and families are now choosing safari vacations in lieu of the more traditional sightseeing tours of Europe’s capitals. The typical quest to observe the Big Five (lion, leopard, rhino, Cape buffalo, and elephant), the striking birds and reptiles, and the extraordinary plant life draws many to African lands. Most of these safaris are planned through tour operators, who offer travelers both the adventure and thrill of the wilderness and the luxury of modern accommodations. These tour operators offer an innumerable variety of scheduled and customized safari options.

East African safaris are typically undertaken in the major game parks in Kenya and Tanzania. Kenya has many game reserves, most notably the Masai Mara, which hosts the start of the annual migration of over one million wildebeest and 200,000 zebras. Tanzania also possesses some of the most famous safari destinations, such as two UNESCO World Heritage Sites—Serengeti National Park and the Ngorongoro Conservation Area—which includes the Ngorongoro Crater, Ruaha National Park, and Selous Game Reserve, and in the north, Tarangire National Park. East African safaris are typically undertaken during the drier months, from mid-December through January or from June to August. Temperatures in the daytime usually range between 70° F–80° F, while mornings and evenings may be quite chilly. Itineraries within these countries may include exotic destinations on the coast, such as Zanzibar in Tanzania and the Kenyan coastal areas around Mombasa and Lamu, or visits to mountainous areas, such as the Usambara Mountains and Mount Kilimanjaro.

In the south of the continent, large game reserves are located in South Africa, Botswana, and Namibia. The Southern African parks can be enjoyed year round, but travelers may prefer the dry season, as during the drier months of April through October daytime temperatures are mild with little humidity and nights are crisp and cool. During this time, game viewing is usually better, because of sparser vegetation. Northwest of Botswana lies the huge expanse of the Okavango Delta and the nearby Chobe National Park. Water activities and more rare types of animals make exploring the Okavango Delta particularly exciting. In Namibia, the Etosha National Park partly comprises a wide salt pan formed 1,000 million years ago. Etosha’s wildlife features the black-faced impala and endangered black rhino, as well as flamingo and pelicans. The South African game reserves are spread around the country. To the east of Johannesburg is the famous Kruger National Park (larger in size than Israel) and satellite reserves in the surrounding area. Many of these safaris include exquisite cuisine and luxurious lodging. In Kwa-Zulu Natal is the Hluhluwe-Umfolozi Game Reserve, the oldest reserve in Africa. This game reserve is famous for being the home of both black and white rhino and many other endangered species. On the southern coast are the Shamwari Game Reserve and the Addo Elephant Park. The private Shamwari Game Reserve supports five ecosystems and for multiple years has been named the World’s Leading Conservation Company and Game Reserve.

For most travelers, the African safari is a relatively low risk undertaking. The actual risk depends on the specific destination, duration of visit, and style of travel. Most of safaris are usually short-term (<2 weeks), guided by experts, and often in a style comparable to western standards. However, accommodations can vary, and the health advisor will need to know if the traveler will be in luxury tented camps and lodges or in more rustic surroundings.

As for travel in any developing country, travelers’ diarrhea remains a major cause of illness. Food and water precautions are important, despite the fact that the traveler may be part of a luxury tour. Other illnesses that are common are minor skin infections, insect bites, suprainfection of bites, and minor respiratory illnesses.

The major risks to the safari-goer are vector-borne diseases, such as malaria and African tick-borne fever. Rarely, African trypanosomiasis has been reported.

Malaria

• Malaria transmission occurs in these areas (except for the southern coastal region of South Africa), and most infections are due to Plasmodium falciparum.
• Chloroquine-resistant P. falciparum is universal.
• Personal protection measures are an important adjunct to malaria chemoprophylaxis. Bed nets are usually supplied at high-end lodges. Permethrin-impregnated bed nets may be required for “do-it-yourself” and budget safaris.

Yellow FeverThe country in which the safari is being taken determines whether yellow fever is a risk and whether the vaccine is recommended.

African Tick-Borne FeverRickettsial infection (Rickettsia africae) is transmitted in rural areas by ticks, primarily in southern Africa. Typhus illness is characterized by fever, sometimes along with localized lymphadenopathy, rash, and presence of an eschar. In addition to using repellents, prevention includes tucking pant legs into socks and doing “tick checks” daily.

African TrypanosomiasisAfrican trypanosomiasis is transmitted by day-biting tsetse flies (Glossina), which are attracted to movement and dark colors. Rarely, cases occur in travelers on safari. Flies may bite through clothing and are often not discouraged by insect repellents. Light-colored clothing is recommended.

EctoparasitesMyiasis (Tumbu Fly)Myiasis is caused by fly larvae penetrating the skin and causing a boil-like lesion with a central aperture. Eggs are usually laid on clothing left to dry out of doors and enter skin when clothing is put on. Clothing should not be placed on the ground out of doors or should be ironed before wearing.

TungiasisTungiasis is caused by direct penetration of skin by a sand flea (Tunga penetrans) that causes a small, painful or pruritic nodule on the foot, often adjacent to a toenail. Wearing closed-toed footwear and avoiding walking barefoot help to prevent this problem.

Waterborne DiseaseSchistosomiaisis (Bilharzia)Infection is widespread throughout Africa. Exposure to freshwater ponds, lakes or rivers, no matter how enticing, should be avoided (see the Schistosomiasis section in Chapter 5).

Other Health and Safety RisksAlthough the above issues are the most important, travelers should be aware of other risks. Rare deaths and severe injuries have been the result of attacks by game animals, especially lions, buffalo, elephants, rhinoceros, snakes, and crocodiles. Prevention includes common-sense behavior, such as remaining inside of game park vehicles unless otherwise directed, avoiding close contact with animals, using flashlights at night, being cautious when exiting accommodations after dark, and not dangling arms and legs in the water while boating.

Robbery and personal injury from crimes are rare occurrences to those on safari because of increased law enforcement practices. However, in some major urban centers, notably Johannesburg, Nairobi, and Mombasa, street muggings during both the day and the night are common. The rates of motor vehicle accidents are among the highest in the world. Within game parks, for the most part, low rates of speed and poor quality roads (in some parks) ensure that high-speed, severe motor vehicle accidents are rare. However, high risk occurs during travel in rural areas between parks, especially after dark.
